JUROR QUALIFICATION QUESTIONNAIRE
If you have received a qualification questionnaire please take the time to read all of the instructions. The form is not a summons for jury service. This form is used to determine whether you meet the qualifications for service as a juror only. If you are deemed qualified to serve as a juror, you may receive a summons to appear for jury service at a later date. There are no longer any automatic exemptions from jury service. All requests for an excuse from service should be raised at the time you actually receive the juror summons.
If you check any of the boxes for questions number 3, 4, 5, 6 or 7 you must provide the appropriate proof as indicated.
If you have questions after reading all of the instructions please call 845-2555 or go to one of our web sites at www.nyjuror.gov, www.wnyjury.com or www.eriejury.com. 10AM
All trial jurors are on call with their qualification/summons and are to report to the courthouse only when instructed to do so by means of a recorded message. The telephone number is 1-800-449-2819.
